The Patient at Risk Program represents a collaboration between 5 different hospital systems across Wisconsin and multiple other public and private partners. It's goal is to be an web-accessible repository for Emergency Information Forms (EIFs) for patients and families across Wisconsin. From our experience, we believe that information can make a difference. Having timely summarized information that is easily accessible wherever the emergency occurs can improve care, decrease delays, reduce unnecessary tests and procedures, can increase the capacity and competency of our current emergency medical system across Wisconsin, and help us better plan for disasters and emergencies.
